Link Services

The Link Program is a Social Prescribing initiative introduced by Healthy Aging Alberta (HAA), which aims to bridge the gap between medical care and social services. The primary goals of Link Services are to connect older adults with the necessary resources to age well in their homes by increasing their knowledge and improving access to community and government services. Services can include navigation and referrals to:

• housing

• financial

• medical

• social/recreational

• legal

• food security

• transportation

• mental health supports

Appointments with the Link Worker are client-focused and empowering, helping individuals build a wrap-around support system in their homes and feel confident in accessing the services they are entitled to. Groups and presentations are also available by request.

Eligibility & Cost

These no-cost services are primarily for adults aged 55+ who are living in Crowsnest Pass and Pincher Creek, and their caregivers who want to learn more about financial or community resources and how to access them.

Link Services cannot provide emergency or crisis support.

Link Services are also available to:

• Organizations that serve seniors

• Professionals wanting social prescribing support

• Caregivers or support workers wanting to learn about resources for older adults in their care

These services are available at no cost to the individual, caregiver, professional, or organization.

Crowsnest Pass:

Mondays from 10:30 am to 2:30 pm

Lethbridge:
1098 3rd Avenue North, Lethbridge, AB
Monday to Friday, 8:30 am–4:30 pm, outside of Pincher Creek and Crowsnest Pass hours
