Program Description

The home support program has been operating since 1959. We continually strive to provide client services that promote autonomy, optimal functioning and health in the most suitable, safe, support and least intrusive environment available. We believe that providing services that enable individuals to remain in their own homes and to participate in the benefits of the broader community not only maintains, enhances and strengthens individual and family life but also is a more cost-effective alternative than institutional placement.

During every contact with client or family, it is an expectation that our staff will respect client choices. Further, the staff is to assist individuals to make independent choices or assume more responsibility for making decisions that will maintain dignity, confidence, and independence. Staff will advocate for clients and family whenever necessary to ensure that they have input into their care, have agreed upon goals and that the authorized service meets both physical and emotional needs of the client and family in both tasks performed and time allotted for service.

Service is provided by certified Health Care Aides who are trained and supervised by a team of experienced individuals including Registered Nurses and Licensed Practical Nurses. Oversight of the program is under the direction of a leadership team made up of Registered Nurses with extensive experience in management, and client care, focusing on seniors and Home Care.

Am I eligible?

In determining eligibility and service levels, an individual support system within the family and community is taken into consideration.

The Individual:
Request for services may be made by anyone; the client, the clients’ family/friends, acquaintances, guardian, Home Care Coordinator, Benefits worker.

Where is the service provided?

In your place of residence within LFS/HS operational areas.

Office hours are Monday – Friday 0830-1630
Client service hours – daily generally between 0700 and 2300, short-term, 24-hour service can be provided
24-hour supervisor on-call access 

How much will it cost?

There is a fee for service, however, if you are eligible the fee may be partially or completely paid by the following:

Program Outcomes

Home Services Mission Statement

LFS Home Services supports individuals so they may independently and safely remain In their homes through personal, respite & palliative care, assistance with all activities of daily living and other individualized services.

Home Services Vision

Promote & Support independent living, thus increasing the opportunity for the individuals to enjoy their preferred living options.

Client outcomes are measured in the areas of Personal care and hygiene, Mobility and aids to daily living, Observations and measurements, Treatments and procedures, Nutrition and meal preparation, Psychosocial functioning, and Respite for the caregivers. Overall deliverables, expected results and short, mid and long-term outcomes are rated in the above areas.
